Health Dimensions Rehabilitation is a Medicare Certified Rehabilitation Agency. HDR's therapists currently provide services in Minnesota, North Dakota, Iowa, and Wisconsin. We provide services in skilled nursing facilities, assisted and independent senior living centers, hospitals, outpatient clinics, schools, pediatric clinics, daily activity centers, and child care centers.
We are proud of our professional team of therapists. We emphasize the importance of consistent staffing in all of our departments. The therapists become an integral part of the team within each facility. Our management team is also responsive to the needs of each individual facility. We are small enough to be able to offer personalized approaches to the establishment of a rehabilitation department, yet large enough to offer a broad base of support and resources.
HDR works closely with the facilities to ensure that each therapy program meets or exceeds the needs of the facility and residents. We select only the most qualified and experiencedtherapists and continually evaluate the needs of each program to make sure there is sufficient staff to meet our high standard of quality care. The hours of operation are set according to program needs.
Long Term Care:
In long term care facilities HDR provides therapy services to the residents as well as to outpatients and home care patients. Our goal is to help our patients return to their highest possible level of independence.
Short Term Care:
Sometimes individuals need intense therapy following a stroke, surgery or an accident. This would require admittance to a Short Stay Unit in a nursing home. Many patients now receive intensive therapy in the short stay unit of their local nursing home. The main objective in short stay units is to return home as soon as possible. Stays average less than 30 days.
Home Care:
HDR is a Licensed Home Care provider. We are able to assist residents in the transition to independent living after being discharged from the hospital or long term care facility. HDR believes that it is important to maintain the continuity of care and to maintain the relationship between patients and therapists. Patients do not need to be homebound in order to receive therapy services in their home.
Alzheimer's Programs:
HDR provides therapy services that complement the programs you currently offer. HDR has worked with many facilities to develop programs that meet the special needs of patients diagnosed with dementia. HDR provides training for our therapists in the use of standardized evaluation tools that can be used to determine the cognitive level of a patient as it relates to functional behavior. HDR's therapists recognize the stages of dementia and assist in the development of care plans that address the most effective approach used for this patient population.
Acute Hospital Services:
HDR provides Occupational Therapy, Physical Therapy and Speech-Language Pathology in several acute care settings. HDR is dedicated to providing therapy in a timely manner to enhance the patient's functional capacity allowing a quick return to independent living.
Outpatient Services:
HDR provides outpatient services to patients from the long term care settings who have returned to independent living but still need therapy. These services are offered in free standing clinics and in clinics located within nursing homes. HDR can and does community marketing of outpatient services in conjunction with the facilities we serve.

Assisted and Independent Living providers are ideally positioned to meet the future needs of our growing elder population.

Pediatric Therapy:
Outpatient Clinic HDR opened “Little Steps”, a free standing pediatric clinic, in 1994. The “Little Steps” clinic, located in Cambridge, Minnesota, offers physical, occupational and speech therapy services to children and young adults from birth to adolescence.

Our therapists have enjoyed great success with their patients in all of these programs for a number of reasons.
The primary reason for their success has been their ability to offer services on site.
We find that many parents simply do not have the resources required to make and keep their children's therapy appointments at outpatient clinics.
Single parents often cannot arrange transportation during the day or they cannot afford to miss work for their child's therapy appointment.
Minnesota school systems are required to provide a certain amount of therapy for children that are developmentally delayed or physically impaired. However, many children do not fall within the narrow criteria that must be met to qualify for services. In addition the schools’ lack of funding limits the amount of services they provide.
Our on site services are also beneficial to the early childhood program itself. They can offer their clients a valuable service at no cost to their program. All the center needs to do is provide our therapists with space to see patients during the hours the therapists are at their facility. HDR obtains all of the authorizations needed, consults with the parents and performs all of the third party billing functions. We also provide all of the equipment and supplies necessary for therapy services.
